The Non-Emergency Medical Transportation (NEMT) Program supports access to healthcare by helping individuals reach important medical appointments when transportation is a barrier. Across the Navajo Nation, distance can delay care, treatment, and follow-up services. This program helps patients, elders, and families connect with healthcare safely, consistently, and on time.

Complete all sections of the application clearly and accurately.

Prepare all required supporting documents before submission.

Step 3: Payment

Payment must be submitted by money order or cashier’s check.

Step 4: Submit

Submit the completed application packet for departmental review.

Listing

  • Navajo Nation Business License and Certification of Incorporation
  • Current Liability Insurance and vehicle registration for all NEMT vehicles
  • Driver licenses, driving records, and criminal background checks
  • Drug test records, First Aid, and CPR certification for all drivers
  • HIPAA policy, complaint contact information, and required program policies
